From what I understand, we will need to share how your agency supports one or more of the 2024 Health Priorities (Mental Health, Housing, Chronic Disease). We will be able to link your scorecard data to the 2024 CHIP. Let me know if you have any questions or concerns. 😊

PFH Microsoft Shared Folder: A space for PFH members to access meeting agenda, minutes, handouts, action team information, and share information with other PFH members. Everyone should have received an email from Microsoft shared files with access to the Partnership for Health Shared Folder. Let me know if you need assistance or don’t have access.
